Physical Examinations

Students registering for Athletics (Dance included), must have a current physical examination on file in the office before the student is authorized to participate in practice. To ensure clearance for participation, physicals should be completed and turned in to the office no later than 3 days prior to the first day of practice.

Physical examinations taken April 1 and thereafter are valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year.

Western Wisconsin Health will be onsite Thursday, July 25th from 9:00am-12:00pm offering sports physicals for $25 in the Ellsworth High School. Parents must be present with student-athletes or provide prior authorization for consent to treat, along with paymnet and all complete forms. To register, please call (715)684-1111.
